Kids Club Fun & Fitness

I hereby authorize Kids Club Fun & Fitness to charge entries on the 15th of each month for services in the UPCOMING MONTH in the Total Monthly Charge amount listed below to my account indicated below and the Credit Card Company named below to charge same to such account. In addition, Kids Club may charge up to $25 additional for registration fees, meet or testing fees associated with each class which may come due during the time period of this agreement. Any credits issued during the previous month will be reflected in a reduced total charged for that month. I acknowledge the origination of credit transactions to my account must comply with the provisions of U.S. law.

By submitting this online form, I understand that I will be billed for the above classes unless I submit a drop form BEFORE the 15th of the month for the next month. Without drop form before the 15th no credit or refund will be available to me.

This authority is to remain in full force and effect until Kids Club Fun & Fitness has received written notice of its termination before the 15th of the month prior to the month's services being billed to afford Kids Club Fun & Fitness a reasonable amount of time and opportunity to act on it.

I also understand that if my card cannot be processed for the amount stated above, I will be charged a $20 late fee that must be paid in addition to the amount due before the 22nd of the month. If not paid by the 22nd , my child will be automatically dropped from class. To re-enter child will have to be re-enrolled and pay all the late fees due.
